Connecting with Patients

Dr. Paris often treats several generations of a family over many years. “He’s seen us through two births, one operation, multiple earaches, a broken wrist and a recovery from a serious traffic crash,” says Jill Farrow, a 43-year-old lawyer whose first visit to Dr. Paris was as a teenager. During the birth of her younger son, Farrow began bleeding badly. Dr. Paris managed to solve the problem in a delicate procedure. “Twenty years ago, she probably would have died,” he says. Today, when he performs school sports physical examinations for the Farrow boys, 10 and 11, he is always reminded that lives are changed forever by doctors just doing their jobs.

To be a mix of country doc and somewhat adventurer, the 55-year-old family physician moved to Hailey after completing his residency(醫(yī)生實(shí)習(xí)期). He hoped to practice medicine there and ski at nearby Sun Valley. Unfortunately, the only job opening was for an emergency-room doctor in Missoula, Montana, 300 miles away. Dr. Paris took it. "I'd ski all day and then drive all night to be in Missoula for a 48-hour shift," he recalls. "I'm lucky to be alive." Knowing he couldn't keep up with his eight-hour commute(通勤), he began taking flying lessons.

In 1981, Dr. Paris joined a small medical practice in Hailey, a former mining town with a population at the time of 2,109. As Hailey grew in the shadow of Sun Valley's booming popularity, Dr. Paris's own practice expanded to seven physicians, including his wife, Kathryn Woods, who is also a family doctor. They met in 1986 at a certification exam in Denver when, in a room full of men in stodgy suits, Woods arrived wearing a Lycra biking outfit and carrying the front wheel of her bicycle (which she couldn't lock up outside). Dr. Paris asked her out on the spot. In 1989, they married.

What Jill Farrow says is to indicate ________.

       A. how weak the bodies of her family

B. how hard it is to be a doctor

       C. how brilliant the physician’s skill is

D. how easy it is to deal with such problems

Dr. Paris often reminds himself that ________.

       A. lives of people should not always stay the same

       B. people can rely on themselves to change their life

       C. doctors should change their own life

       D. it is the duty of a doctor to heal the patients

Why did Dr. Paris move to Hailey?

       A. Because he can be a doctor and an adventurer there.

B. Because he has to finish his residency there.

       C. Because his children are fond of skiing at nearby Sun Valley.

       D. Because he has to be an emergency-room doctor there.

We can infer the doctor got married probably at the age of ________.

       A. 27.                   B. 37.                   C. 17.                          D. 47.

Abuse can be defined as “to treat wrongfully or harmfully”. There are different categories of abuse that have been recognized and within our case study there appear to be two distinct forms of abuse, family abuse and child abuse. These can be subdivided into terms of physical abuse and psychological abuse. Physical abuse is the intentional physical injury or harm or deliberately not preventing harm occurring. The minimum physical signs seen in our study of James are bruises but they might feel more painful to his heart than the more serious invisible injuries. Emotional abuse is the continual failure to meet basic emotional needs. Emotional development is prevented and well-being harmed. The emotional signs in our case study can be seen in James by his actions of being withdrawn and non-communicative. The behavioural sign to abuse taking place to James is his aggressive behaviour.

The short term effect of physical abuse on James is physical pain. In the long term, injuries that often happen can result in secondary illness and complications, permanent scars or even disabilities. His emotional effects in the short term are a fear of people, withdrawal, and poor relationship with others. The long term emotional effects could be low self-respect, depression, inability to form relationships.

Abuse can arise for many reasons and there are a number of theoretical views which may be useful in clarifying why the abuse has taken place. The female view believes that sex and family roles give approval to a culture of abuse. Considering the historical and fixed ideas, men have power and control. In James’ case he lives in a re-constructed family with the father being rude and a heavy drinker. From a psychological angle, alcohol misuse can bring mental health problems which may increase aggression in the person and so James is more at risk from abuse by his stepfather. The relationship between the mother and James involves a dependency of James on his mother. With other problems in James’ mother’s life, this leads to increasing stress and the inability of his mother to cope with and manage a family with four children borne by two mothers. The attachment theory states that significant separations of a child from the carer in the early years can have an effect on their emotional development and can lead to psychological and social difficulties in later life. The loss of both his father and his half sister’s father with whom he lives may have contributed to his behaviour.

Abusive behaviour can sometimes be the result of mental health problems, brain damage or being abused themselves. By becoming the abuser they believe they are taking control; some even believe that they are not doing anything wrong and cannot stop themselves. When working with individuals who have abused, it is important to be aware that they may go on to abuse again and there is a need to protect the community from the abuser.

The college entrance exam is not only a big challenge (挑戰(zhàn)) for Chinese high school students, but also a very important exam in the lives of South Korean students.

Although the long, cold winter has already started in South Korea, the annual (一年一度的) national exams have made the atmosphere very heated.

More than 675,000 South Korean high school graduates took the college entrance exam last Wednesday. They usually take exams in Korean, maths, sociology (社會(huì)學(xué)), history and foreign languages.

Officially there is one college place for every 1.33 students. But because all the students want to go to the top universities in the country, the competition can reach one place for 10 students. The students want to attend these colleges for both their famous names and better job opportunities.

Because students face fierce competition, they have to study very hard to realize their dreams. Park Seung said he and his classmates often go to school before 7:30 a.m. After school has finished at 6:30 p.m., most of them go to the library to continue their study instead of returning home. Since many libraries in South Korea are open 24 hours a day, they often stay long into the night. Many of the Senior 3 students only sleep for three to four hours a day. “I feel a lot of pressure, but I have to study very hard in order to make my dream come true. This is my lifetime goal and it will be a turning point in my life which could decide my future,” Park said.

The exam day is a very serious day for the whole of South Korea. Vehicles are not allowed within a 200-metre radius (范圍) of all the test sites to make sure the students have quiet surroundings. Tooting (吹奏) of horns is forbidden, even airplanes are ordered to avoid landing and take-off near the test sites during listening comprehension test hours.

Students are told their scores in December before they apply for college. This is followed by face-to-face oral tests. There are public and private universities in South Korea. Many private universities are well-known, but their fees can be 18, 000 yuan each term. This has made a lot of students think again.
